EXPANDED FUNCTION DENTAL AUXILIARY

The Expanded Function Dental Auxiliary program only admits students from a waitlist of qualified candidates. All prospective students are required to complete this form and supply all the required information and documents to be considered. Qualified applicants are added to the waitlist on a first come, first served basis, and admission is prioritized based on DANB-CDA exam results or graduation from a CODA accredited Dental Assisting program. It is strongly recommended that students seek advising from the Expanded Function Dental Auxiliary program director prior to being added to the waitlist.

Please remember that we will use the contact information provided on this form so please ensure it is current so we can reach you.
